About the role

Join our welcoming and supportive team at Estia Health Reservoir, a warm and vibrant 120-bed home located in Melbourne’s north, just 12km from the CBD and nestled between Preston and Bundoora. With easy access to local shops, cafes, parks, and public transport, this Full Time Clinical Care Coordinator role offers consistent Full Time hours Wednesday to Sunday. 

A comfort to our residents and a leader to the team – being a Clinical Care Coordinator is all about how you make people feel.  You will:

• Driving a continuous improvement culture
• Acting as a clinical resource for staff, offering clinical education
• Focusing on clinical service delivery, compliance, and audit processes
• Care planning in line with new standards
• Taking pride in your clinical documentation management skills

You’ll be someone who is empathetic and who’s previously worked in an aged care home. You’ll also have:

• A real desire to make a difference in people’s lives
• Leadership experience in clinical care
• A nursing degree and current AHPRA registration
• Solid documentation skills and knowledge of AN-ACC
